Submission Information
If you are interested in trying out for any of our varsity athletic programs, you must first complete the "Required Physical Forms" found below. These forms can be completed by your family physician and mailed (must be turned in before practicing) to the Athletic Trainer or can be completed by our Penn State Harrisburg physican at the on campus physicals. These forms are required before any tryout, practice or competition process begins. Please send completed physical forms to: Penn State Harrisburg, ATTN: Athletic Trainer, 777 West Harrisburg Pike, Middletown, PA 17057. DO NOT SEND PHYSICAL FORMS TO HEAD COACHES - THEY MUST BE SENT TO AN ATHLETIC TRAINER. If you have any questions regarding preliminary requirements, please contact the athletic department at 717-948-6744.
REMINDER: Outside physicals must be dated within 90 days from the beginning of the semester to be accepted.
First-Year Student Athletes: Required Physical Forms
*All first year students trying out and/or participating on a varsity athletic team must complete the forms below.
** Mandatory additional paperwork must be completed through frontrush
- All first-year student-athletes will need to have a full physical and complete this form.
- All first-year student-athletes are required to provide a copy of their Sickle Cell test. Please email head athletic trainer Pam MacNett-Kennelly at pmm25@psu.edu with any questions.
Returning Student Athletes: Required Physical Forms
*All returning students trying out and/or participating on a varsity athletic team must complete the forms below.
** Mandatory additional paperwork must be completed through frontrush
- If student did a full physical for the 2022-2023 season, please complete the recertification form.
- If you did a recertification form for the 2022-2023 season, you will need to have a full physical and complete this form.
Affidavit of No Insurance
In the case that a student athlete at Penn State Harrisburg does not have his or her own personal or family medical insurance, the below affidavit of no insurance form must be on file for the student athlete. This form must be signed in front of a Notary Public and turned in with the above "Required Physical Forms". The form is required before any tryout, practice or competition process begins.
